The information and authorizations on these forms are necessary for us to be able to provide care for your animals. Your security is one of our primary concerns, and this information will be stored securely in our practice. We do not sell our clients’ information.

Payment Terms
Payment is due at the time of service. We accept cash, checks and major credit cards both in the field and at the clinic, and we accept Care Credit cards at the clinic or over the phone. We also offer an autopay system with a credit card or Care Credit card on file. For autopay, invoices are paid with the on-file credit card within 1-7 days of service, and your invoice is emailed to you.
We do not have a policy of carrying client balances. If you foresee needing a payment plan for care, please use a credit card or Care Credit card for payment in full.
For the security of your personal information and data, please send the forms to us by US Mail or by fax, or bring them to your appointment in our office or at your barn. We do not recommend email.
